Required Attachments
Include ONE of each of the following, as applicable, with your application materials.
- Cover letter from the Chief Executive Officer endorsing the proposal, summarizing the background leading to the project and explaining how it relates to the organization’s mission and priorities. The letter should also contain a paragraph confirming that all due progress or final reports have been submitted on any past Murdock Trust supported project(s). If not, the Trust should be contacted for forms to use for such report(s). The CEO is also responsible for Item 2 below.
- Required information from the Internal Revenue Service (IRS)
- IRS document showing your 501(c)(3) tax exemption ruling for the current legal name
- IRS document showing your 509(a) non-private foundation ruling for the current legal name (often, not always, these two rulings appear in the same IRS document)
- Certification in a currently dated and signed letter from the Chief Executive Officer that the submitted IRS rulings (above) are still correct and that no modifications are planned or pending. Please refer to both rulings in your letter.
- If there is a recent name change, please supply IRS recognition of that change.
- If your organization is tax exempt under a group ruling for a central organization, attach the latest certificate of membership or evidence of current affiliation.
- If tax exempt as a government entity or tribal government, use the appropriate generic letter for Government Entity Status or Indian Tribe Status to certify your tax exempt status.
- List of names, addresses and professional affiliations of your Board of Directors.
- Names, titles, addresses and telephone numbers of professional references qualified to evaluate this project. Also, any recent newsletter, news articles or special brochure describing the project and/or campaign. Include any outside letters of endorsement. Include your most recent annual report.
- One to three different 8-1/2” x 11” drawings of any facility and floor plan to be constructed or renovated with the requested funds from the Trust and other donors.
- Most current monthly financial statements together with budget comparisons; current fiscal year budget; your spending policy regarding any endowment funds; cash flow forecasts with and without requested Murdock funds; any long-term financial plan.
- Complete copy of the most recent audited or reviewed financial statements or most recently submitted IRS Form 990 (if the audited statements are not available).
- If available, an executive summary of your current organizational strategic plan, showing the relationship of this project to your plan.
