Grantee Reports

The Trust expects to be kept fully informed of progress on your project and the expenditure of the grant award. Progress reports, requests for payment and final reports should be made using these forms. Please be sure to download, complete and submit the form that corresponds with your specific project as instructed by the Trust’s staff. Once you have completed and signed the appropriate form return it by e-mail or mail it to the Trust’s Office. If you have questions, please contact your assigned program director.

	Certification of Meeting Grant Conditions This form is used when the grant contingency has been met and the grantee is requesting payment on all or part of the contingency amount. This form is usually used in conjunction with the Progress Report and Payment Request Form.
	Certification of Meeting Grant Conditions for Research Grants This form is used when the scientific research grant contingency has been met and the grantee is requesting payment on all or part of the contingency amount. This form is usually used in conjunction with the Progress Report and Payment Request for Research Form.
	Progress Report and Payment Request Form The Progress Report and Payment Request form is used to request payments and should be received at the Trust no later than the first of the month the payment is to be scheduled.
	Progress Report and Payment Request Form for Research The Progress Report and Payment Request for Research form is used to request payments for scientific research grants and should be received at the Trust no later than the first of the month the payment is to be scheduled.
	Final Report Form & Certification of Funds This form is used to report final distribution of all grant funds expended and is due no later than one year after the final payment of grant funds has been paid to the grantee organization.
	Final Report Form & Certification of Funds for Research This form is used to report final distribution of all scientific research grant funds expended and is due no later than one year after the final payment of grant funds has been paid to the grantee organization.
	Annual Report for Partners in Science This form is used to submit an annual report for the Partners program, to be completed by the Mentor and Partner, and is due September 15 after each summer of research.
	Final Financial Report for Partners in Science This form is used to submit a final financial report for the Partners program, to be completed by the Mentor’s institution, and is due May 1 after the second summer of research and after the Partner has attended two January national conferences.
	Annual Report for Partners in Science Supplemental This form is used to submit an annual report for the Partners supplemental program and is due May 1 of each year.
	Final Report for Partners in Science Supplemental This form is used to submit a final report for the Partners supplemental program and is due May 1 after the high school teacher has attended two January national conferences.
