Grantee Reports

The Trust expects to be kept fully informed of progress on your project and the expenditure of the grant award. Progress reports, requests for payment and final reports should be made using these forms. Please be sure to download, complete and submit the form that corresponds with your specific project as instructed by the Trust’s staff. Once you have completed and signed the appropriate form return it by e-mail or mail it to the Trust’s Office. If you have questions, please contact your assigned program director.

This form is used when the grant contingency has been met and the grantee is requesting payment on all or part of the contingency amount. This form is usually used in conjunction with the Progress Report and Payment Request Form.

Annual Report for Partners in Science

This form is used to submit an annual report for the Partners program, to be completed by the Mentor and Partner, and is due September 15 after each summer of research.

This form is used to submit a final financial report for the Partners program, to be completed by the Mentor’s institution, and is due May 1 after the second summer of research and after the Partner has attended two January national conferences.
